We have adopted the following Privacy Policy for the FAIR Health website (fairhealth.org) and all successor sites, including, but not limited to, the FH Consumer Cost Lookup website, the FAIR Health Research Support Program website and the FH Fee Estimator website (collectively, the "Site"). The Site is owned and operated by FAIR Health, Inc. (“FAIR Health,” "we," "us," "our"). By using the Site you, as an individual and on behalf of your company or institution (“you,” “your”) agree to the collection and use of your personal information by us in accordance with the terms of this Privacy Policy.

This Privacy Policy is current as of the effective date stated above.  We reserve the right to change the terms of this Privacy Policy at any time without prior notice. Any changes will be posted on this page; therefore, we encourage you to check this page regularly. You can determine when this Policy was last revised by referring to the "DATE EFFECTIVE" legend at the top of this page. Your continued use of the Site following any changes to this Privacy Policy will constitute your acceptance of such changes.

Information We Collect

You can visit and browse the Site without revealing your own personally identifiable information. You also may voluntarily disclose additional information to us, which may include, but is not limited to:

If we receive actual knowledge that we have received such personal information, we will delete the information from our database as soon as possible.

We do not disclose any personally identifiable information that we collect from the Site to third parties for their marketing purposes without your prior consent.

We may disclose personally identifiable information to third-party service companies to help us operate our business and the Site or to administer activities on our behalf and may share your information with these companies for those limited purposes.

We use non-personally identifiable information for internal purposes and to help us interact with visitors to the Site. These safeguards are regularly reviewed to protect against unauthorized access, disclosure and improper use of your information, and to maintain the accuracy and integrity of that data.  Unfortunately, no data transmission over the Internet or data storage system can be guaranteed to be 100% secure. As a result, while we are committed to protecting your personally identifiable information, we cannot ensure or warrant the security of any information you transmit to us through or in connection with the Site or of information stored by us.
